Hello, vur grade 4 requires surgical correction & ureteric reimplantation if needed. Paediatric surgeon along, with pediatric nephrologist are best to take that decision. Since its an anatomical problem, medicines have no role. Antibiotic drops r given to her as a chemoprophylaxis to prevent recurrent, urinary tract infections, to which these children are prone to get.

Hi vur may require regular routine and culture of urine in case of fever. And phexin prophylaxis may have to used for 6-12 months. However it is unlikely to subside my medical management. You may ask your urologist for botulin toxin or directly go for surgical approach. Post surgery if there is no fever / uti for a year unlikely that kidney may have any residual underperformance.
