I am 21 years female. My stomach is burning after eating and sometimes pain for nearly 3 months. My stomach is even bloating. I took ultrasound scan it is normal and I have done endoscopy. The report showed large hiatus hernia 35 cm. Gastrosurgeon provided gastric tablets like esomeprazole. But the symptoms reoccur when I don't take gastric tablets. What to do? Can you suggest what might be the problem?

Hi lybrate-user, if you have been diagnosed with hiatus hernia (symptomatic) and not getting relief with medications, then probably you will need fundoplication surgery. It is better to get the surgery done in a centre who are doing this type of surgery regularly with excellent result. Till that time, take digeraft liquid. 2 tsp three times daily and esomeprazole + itopride combination cap. Hopefully it will provide you some relief. I would try to answer your queries the best I can. * first of all let us understand what is hiatus hernia. When you swallow food after chewing, it goes through the food pipe which we call "esophagus" the esophagus passes though your chest and enters the abdomen via a small opening (in the diaphragm which is a thin muscle separating chest and abdomen) and then joins the stomach. Now the stomach produces acid which helps in breaking down food so that it can be digested easily. Usually, this acidic content from stomach only moves forward towards the intestine. It cannot go back into the esophagus. However, in case of hiatus hernia, this acidic content can go back up into the esophagus. This causes irritation in the esophagus leading to burning and pain. Similarly it also causes feeling of bloating or fullness soon after taking a meal. * the medical treatment for this problem includes (1) drugs like esomeprazole that reduce acid production in the stomach, thus providing relief from the burning sensation, (2) drugs like esomeprazole when combined with prokinetic agents are more effective as they help move the food from stomach to intestine faster than usual thus there is less content in stomach to regurgitate back to esophagus, (3) antacids can also be used for the same reason as they neutralize the acid in stomach. * along with medications or if you want to avoid taking medications, there are several lifestyle changes that can help: (i) avoiding spicy food, fatty-fried foods, coffee, tea, alcohol, smoking (ii) avoiding heavy meals, specially before bed time. Take frequent small meals instead. Having an early dinner is beneficial. (iii) exercise is helpful. Maintaining a healthy weight is quite beneficial.
