Azoospermia, which means the absence of sperm in the semen, can be categorized into two types: obstructive and non-obstructive. Obstructive azoospermia occurs when there is a blockage in the reproductive tract that prevents sperm from being ejaculated, while non-obstructive azoospermia is associated with a lack of sperm production in the testicles. Here are potential options based on the type of azoospermia: obstructive azoospermia: surgical repair (microsurgical epididymal sperm aspiration - mesa or testicular sperm aspiration - tesa): in cases where the obstruction is due to a blockage in the reproductive tract, surgical procedures can be performed to retrieve sperm directly from the epididymis or testicles. Vasectomy reversal: if the azoospermia is a result of a previous vasectomy, a vasectomy reversal may be considered to restore the flow of sperm. Non-obstructive azoospermia: hormonal therapy: hormonal treatments may be considered if the azoospermia is related to hormonal imbalances. This could include the administration of hormones such as fsh (follicle-stimulating hormone) and lh (luteinizing hormone) to stimulate sperm production. Micro-tese (microscopic testicular sperm extraction): in cases of non-obstructive azoospermia, where the testicles are not producing sperm, a procedure called micro-tese may be performed. This involves the surgical extraction of small amounts of testicular tissue to identify and retrieve viable sperm for use in assisted reproductive techniques. Sperm donor: if no sperm can be retrieved from the individual, the option of using donor sperm in assisted reproductive technologies, such as in vitro fertilization (ivf) with intracytoplasmic sperm injection (icsi), may be considered.
