Doctor, I am suffering from ring worms on inner thighs, hips, near penis from a month. I have been using ring guard for 20 days but no relief. Please help me.

Never use ring guard as it contains salicylic acid which irritates skin and causes scarring and thickening of skin and its effect on fungus is not specific. You are having fungal infection. Fungus may be Candida albicans or dermatophytes or both. Moreover when fungus is in vagina or vulva or on glans & prepuce, you may also have trichinosis as well as infection with several low virulent bacteria like Ureaplasma urealyticum. Fungus is a plant and grows rapidly in hor humid environment. So it is more in raining season. If sweating occurs frequently, it again rapidly grows. If he has wetness in prepuce or she has wetness in her vagina due to overstimulation or due to any reason, again it grows rapidly and difficult to eradicate. Fungus produces lacs of spores when you try to eradicate it and no medicine can kill spores. We can kill only baby filaments coming out of spores. For that we have to give antifungal on and off for long times so all the filaments coming out of spores get killed and no intact spore remains there. Itching is due to allergic response to fungal filaments and spores. Spores cannot be killed but can be washed off with soap & water washing. Now how to eradicate it? Wash the infected parts & private parts 2-3 times daily with mild soap or with Cetrim soap. Better take bath at least 2 times daily Dry the infected & private parts in fan's wind (or hot blowers wind in cold climate) before wearing clothes. Buy a small table fan dry parts with that. Keep 2 small towels (soft ones, better parts of old sootee dhoti but well cleaned) with you always. Whenever you get sweating over your private parts, take one towel, wet it with water and swipe on your private parts. Afterthat use another dry towel, and scrub the parts. Then dry the parts in fan's wind or by blowing wind with some thin copy or newspaper or old manual fan. To keep parts dry is very essential Wear only loose aerated thin cotton clothes. Use Clotrimazole+Betamethasone cream like Candid-B 2-3 times daily after washing and drying the parts. DK gel contains Micanazole which is not as effective as Clotrimazole. Use Candid Vaginal tablets to insert in vagina in case female. In outer parts of vagina (vulva), use Candid-B cream. Also use Candid-B cream on penis during intercourse. Take Fluconazole 150 mg once daily for 3 days and then once weekly (better on every Sunday) for 5-6 months Take Vitamin A preparation like Aquasol (25000 iu) once daily for 7 days followed by Becadexamin once daily for 2-3 months. Vitamin A is very essential. It causes rapid growth of epidermal layers of skin creating new skin underneath so that fungal infected superficial layers get shed off rapidly and fungal eradication occurs very fast For viginal and penile infection, better use Nuforce 3Kit containing Fluconazole 150 mg ?(for Fungus), Azithromycin 1gm (for Ureaplasma uralyticum) and Ornidazole 2gm (for Trichinsis), one kit every Sunday for 2-3 weeks. Give Kit to both the partner simultaneously. You may also use Fluconazole shampoo and Selsun shampoo along with above treatment if you have dandruff or fungal infection on whole body. Other antifungals like Terbinafine, Itraconazole, Amphotericin-B are not required, they are for systemic fungal infections.
