Hemifacial spasm is a nervous system disorder in which the muscles on one side of your face twitch involuntarily. Hemifacial spasm is most often caused by a blood vessel touching or pulsating against a facial nerve. It may also be caused by a facial nerve injury or a tumor. Sometimes there is no known cause. Hemifacial spasms aren't dangerous on their own. But a constant twitch in your face can be frustrating or uncomfortable. In severe cases, these spasms can limit function due to involuntary eye closing or the impact they have on speaking. Or most people, botulinum toxin injections are the most effective treatment for hemifacial spasm. A healthcare provider uses a tiny needle to inject a small amount of botulinum toxin around affected facial muscles. These injections temporarily weaken the muscles and stop the spasms. When should I worry about face twitching? There are several scenarios in which you should consult a doctor: if your twitching doesn't resolve after several weeks, if it intensifies or interferes with your vision, or if you simultaneously experience other significant symptoms, such as a headache or muscle weakness.

Hello, thanks for the query. In a patient with long standing diabetes peripheral neuropathy is quite common. This can happen even with a fairly good control. Because high blood glucose affects peripheral nerve functions. Treatment is to achieve strict glucose control (fbg < 100 mg, pp 150 to 160 mg & hba1c% <6.5%), plus take drugs that are supposed to be helping in improving nerve function and control the symptoms. Drugs can be b12, lipoleic acid, pragabalin and/ or gabapentine and combinations of these. Another important aspect is with a bmi (body mass index) f 29.43 kg/sq meter (normal < 23 kg/sq mete) you are in grade one obesity with an excess of over 20 kgs. This does affect achieving better glucose control. Therefore, it is absolutely essential to achieve stric glucose control and then look at other drugs. Thanks.

Hello, thanks for the query. Usually in a person with long standing diabetes mellitus, symptoms of periphera; neiropathy develop. The condition is seen in most of the diabetes patients, particularly it is worse in those who had a badly controlled blood glucose levels and high hba1c%. If diabetes is controlled well right from beginning then the severity is quite less. However, if the control is persistently bad then severity of symptoms more. Therefore, apart from achieving a very strict glucose control, various medications that are mentioned need to be taken. Since you have both sensory and motor effects it is a difficult situation. You will have to continue the medications and hope it will improve. Thanks.

Hello, it appears that that though you have been diagnosed with sensory axonal neuropathy, the cause has not been detected. Gabawin (pregabalin) is used for relieving the pain associated with the neuropathy, I do not know whether you are having pain or owhat are the other symptoms. So if you are not having pain arising from diseased nerves its not going to help. Neurobione is probably helping by repiar of nerves and should be ok. With thr details you have provided thats all I can say.

The neuropathy which is due to deficiency of vit b12 will benefit most. As you have not mentinoed the cause and signs and symptoms of the neuropathy you have its difficult to comment.
