You have been operated for mitral valve replacement for which you are taking an anticoagulant (noac or novel oral anticoagulant, so it requires dose adjustments from time to time. Because if it goes above 3.5 you'll be having high chances of internal or traumatic bleeding. If it goes low you'll be having increased chances of clot formation in the blood.
