The response to chemotherapy is the best. Immunotherapy is reserved for those who are not candidates for chemotherapy or those whose disease progressed on chemotherapy. If you are not willing for chemotherapy, then the immunotherapy may be tried. But the cost of immunotherapy is around Rs 2 lacs per month and it needs to be given for atleast 6 months to know if response is there and if good response then need to continue the same till disease progression. I would be in a better position if you send me the details of the treatment given so far and the reports of radiology and pathology. For lung cancer (NSCLC) we prefer surgery for solitary lesion (it gives pathology which SRS cannot), or for lesions causing raised intracranial tension with risk of brain stem herniation. For those with multiple lesions without raised intracranial tension, if mutations are present, targeted therapy is first choice. For those without mutations, SRS is preferred.
