Your father had already undergone a surgery for tongue cancer. But even after surgery lymph nodes have come positive in the recent pet scan. So the tumour has metastasis in the lymph nodes which are showing up now. There is no definitive thing as to which will work surgery or chemo. Chemo if used now will be just for palliation that is it may extend your father's life but it will not cure the disease. If you'll talk about surgery. It will cure the disease but who knows how many more nodes or organs has this tumour extended to. So it all depends on the surgeon's preference and the patient's age and functional disability that occurs after surgery.
