It looks to me that you are suffering from proctalgia fugax. Try warm sitz baths twice a day. If that doesn't help please consult a surgeon and have yourself examined by him. Medical management can be decided after that examination only. Thanks.

I would like to know on what basis was a diagnosis of proctalgia fugax was made. Without a proper clinical examination it is difficult to make a diagnosis and an MRI may be required to clear all doubts.

This could be a transient blurring of vision. It may be innocuous but could point to some other disease like giant cell arteritis, cerebrovascular ischemia, retinal arteriolar emboli, and amaurosis fugax syndrome. Transient visual loss can therefore be a symptom of a serious vision or life-threatening condition, requiring urgent investigation and treatment.

I'm unable to conclude by the term uneven surfaces in anorectal region, it could be internal piles or a growth, you need to visit a proctologist to have a proper diagnosis for proper targeted treatment. If proctologist does not find any cause, then you may be having proctalgia fugax, where some have pain inspite of not having any problems. I'm sorry I can't prescribe any medicine except xylocaine 5% gel/ ointment for local application with applicator until you have a confirmed diagnosis.
