May be his fissure has not healed properly even after surgery. This is not unusual. An alternate treatment which will help in promoting healing of Anal Fissure should be adopted for him. He is advised to have Sitz bath and apply Sushruta Ointment+ Lekhan Powder twice daily and administer Sushruta oil in night. For easy evacuation of bowel; he can take some stool softening Ayurvedic medicine like Sushruta Churna. You can get these medicines through courier. Review after 15 days.

Well. Fissure doesn't usually recur if a sphincterotomy is done properly. However you will always need to avoid constipation as it may cause some discomfort and trauma. Regards.

Some people are prone for recurrence while others might remain disease free after surgery for a long time. Till yet there is no way to predict. Who is gonna fall in which of these two groups.

As you have mentioned that you got surgery for pilonidal sinus this shows that you have more hair then normal. I simply suggest you to keep cleanliness of area by washing with soap and water and apply some moisturiser or home remedies coconut oil. Another suggestion is to keep your stool soft by taking three full meals which has lot of veg. Fruits and plenty of water and if needed add stool softener to it. i hope you will improve.
