Anal Fistula is a tunnel like communication structure between anal canal and outer skin. There are certain glands in anus whose secretions keep the anal passage lubricated. If due to any reason infectious agent invades these glands abscess formation occurs which subsequently spreads towards the skin and bursts outside forming an external opening.

May be his fissure has not healed properly even after surgery. This is not unusual. An alternate treatment which will help in promoting healing of Anal Fissure should be adopted for him. He is advised to have Sitz bath and apply Sushruta Ointment+ Lekhan Powder twice daily and administer Sushruta oil in night. Well. Fissure doesn't usually recur if a sphincterotomy is done properly. However you will always need to avoid constipation as it may cause some discomfort and trauma. Regards.
