Hi lybrate-user, you are suffering from urethral stricture disease, by your history it is obvious that you have undergone viu or visual internal urethrotomy which is a temporary procedure to keep your urethra open. This is a reason why you have been advised self catheterization. You are bleeding while inserting catheter because your stricture has recurred. The only permanent solution to you problem is urethroplasty which will give you long term results.

For curing it completely you need to do bladder retraining exercises. You need to increase duration of passing urine by 5 minutes daily till you can control it for 2-3 hours. Continue dariten or you can try mirabegron 25 mg twice daily for 1 month and see how is the response.

For a person with diabetes it is very essential to achieve a strict glucose control with fasting < 100 mg, PP 160 to 170 mg, HbA1c% <7. If glucose levels are higher than those mentioned there is likely hood of glycosuria, hence frequent urination. Since there is no mention of his actual glucose levels, it is difficult to comment. Thanks.

I think his whole symptoms are because of stone in ureter. Meet another urologist and get that ureteric stone removed. He should be alright.
