Hi, generally fistula doesn't cause pain. It may be a fissure or a perianal abscess. Just consult your doctor again. Get a Trans anal Ultrasound done to get a proper diagnosis.

Usually following any surgery a patient needs about 10 to 15 days to recover. As regards the cost it depends on the type of procedure, surgeon and the hospital that one is going. Because in government hospitals cost will be much less. Difficult to give a figure. Which surgery will be decided by the surgeon, based on the specific situation. Call on any good surgeon he should be able to do that.
