Its always better to get a checkup done so as to confirm what the exact diagnosis is. In majority of cases the waist pain may be due to lumbar disc problem or spondylosis. Other causes may be renal problems. A renal profile test along with urine routine test and USG abdomen may help detect the kidney problem if present.

Regular alcohol intake in moderate quantity or occasionally usually does not damage kidneys so badly and rapidly leading to failure. There must be some other cause for kidney failure in your friend. Causes for kidney failure are long standing diabetes (20 plus yrs), not well controlled, high bp again of long standing, kidney infections or other diseases of kidney. Usual symptoms may be swelling of lower limbs, reduced urination, passing of albumin in urine etc.

If both the kidney are damaged and don't function than we do dialysis which functions like a kidney and filters the toxin.
