My father is 73 years old, has pacemaker implanted, suffers from hypertension, takes medicine Telma 40. Recently,on two occasion he got sick, felt redness ,not able to walk, while on road and on checking ,BP was found as low as 87/46 etc. Further checking in home for two days revealed that in the morning his BP remains low like ,99/70 and in night like 169/99. Recently he got a wound and for that he had to take tedvac and medicines like cefixime 200+,Linezolid600, Chy moral forte, bifilac,amoxicillin and cloxacillin 625 etc as per advice of two different doctors. Now ,What may be the reason behind such BP fluctuation and what should be the treatment?

1) This BP fluctuation can be due to stress of infection for which he is being treated. It is common to find such extreme variations in BP in infections and hence the dose requirements of BP medicine changes. Probably taking the routine dose of medication is causing a sudden fall in his BP and his body reflexes are not able to respond to it leading to weakness and falls called as Vaso-Vagal syncope. 2) My suggestion is please get him thoroughly evaluated for infection and treated for that as well as consult your cardiologist for a pacemaker interrogation, echo and change of medication.

Change dose to 20 mg twice daily. Measure BP only after patient has rested for 10 minutes. Skip evening dose occasionally when patient having fever or upset stomach.
