[08/02, 8: 11 p.m.] hlo, sir I am a type 2 diabetic patient since 1996. I am a male of 58 years old and suffering from high bp, prostate, and ckd (since 2016 ). I am taking the following medications since 2015 :- 1) lantus inj - 5 units- overnight before dinner. 2)cilaheart 20 - everyday before breakfast. 3) zita met plus 20/500 - overnight before dinner. 4) zoryl m3 forte -everyday before breakfast. 5) tazloc -everyday before dinner. 6) nefrosave -everyday before breakfast. 7) dytor 10 mg - everyday before breakfast. 8) jardiance 25 mg-once a day before breakfast. 9) dariton od 7.5 mg -twice a week.[08/02, 8: 20 p.m.] my sugar levels always remains quite high even after time to time medications and fibrous & balanced died. Last one year ago, I suffered from hypoglycemia and there was some blood clot in my brain but it was operated successfully. I was also treated with laser retinopathy twice since 5 years back. *problems*:- (1) after treatment of blood clot in my brain, I am suffering from frequent _short term memory loss_. I even forget things of last 5-10 minutes back. (2) I feel heavy weakness & energy loss.[08/02, 8: 23 p.m.] sagrika: *current levels :-* (1) blood creatinine-1.98 (2) prostate weight - 40 gms (3) hba1c - 8.1 (4) egfr- 49 [08/02, 8: 23 p.m.] *please reply a humble request.*
