At present I am taking bp medicines as under- stamlo 10 mg once a day, cardivas 25 mg twice a day, minipress xl 2.8 mg twice a day and arkamin 100 mcg fpr the last 8 years. But my pressure at present is in the range of 150-17- and 70-80 at sitting and supine position.in other words my systolic pressure always remains at a higher range. Pl review the above medicine and a switch over plan for effective control of blood pressure. One thing you may observe in my present medicine, there is dominance of alpha blocker. Pl advise.

This is what is called as refractory hypertension requiring multi drug therapy. A detailed evaluation to look into any correctable causes is very important in your case. Disorders like obstructive sleep apnoea, hypothyroidism, renal diseases must be ruled out. I would suggest you to visit a cardiologist nearby and get yourself evaluated. Also please get a renal arterial doppler done. Renal artery stenosis or in simple words blockage in vessels of the kidney is one correctable cause of hypertension in patients with refractory hypertension. Also you need to make certain lifestyle changes like smoking and alcohol cessation if any, regular exercise to loose excess weight if any, decrease dietary salt consumption by avoiding packaged food items. In the meanwhile I suggest you increase the dosage of stamlo to 10 mg twice a day, continue rest of the medications as before. Get yourself evaluated, its important, I hope you understand.
