I am having diabetes (prenatal) from 15 years. I am taking medicine tab duotrol twice a day and insulin novomix 15 unit per day. 4-5 months earlier my fasting sugar was 160-180 range but as of now it become 200 in fasting and 240 on pp after two hour of meal. I have not any complication but I am worried.

Duotrol consists of 5 mg of gilbenclamide and 500 mg of metformin. If I had consulted a patient in person with such an issue, I would have done two things. 1. Check the hba1c 2. Increase the dose of metformin (added separately) to 1000 mg twice daily (the maximum dose of metformin is 2500 mg per day in divided doses). Metformin is a very good insulin sensitiser and it overcomes insulin resistance. There no chances of hypoglycemia with metformin along and chances are high that one's insulin requirement might come down.
