I?ve had ocd since I was 13. Most of the obsessions were study related, though I had some normal compulsions too. I started treatment in 2017, I was prescribed fluvoxamine (20 mg) and cbt. I attended some therapy sessions while I was taking the medicines. It seemed to me like the medicines were working. It affected the compulsions. It was?t as severe as it was before but, my obsessive thoughts about things important to me appeared to be unchanged. Then I discontinued therapy but kept taking the medicines. During that phase, I could?t see any appreciable improvement during my obsessions. So I discontinued medication. I restarted treatment again in march 2019, as my obsessions were still there. The dosage was increased and I made some improvement i.e. Less anxiety. It?s not getting improving now though. I still have obsessions about things important to me. The uneasy anxiety is still there. How do I know if my condition is getting better? Do I need therapy? Are the medicines working?

Dear Samarth, You can go for YBOCS scale to assess your improvement. Feeling less anxious and no compulsions are improvements with the tablets. You may go for adequate dose of Fluvoxamine or CBT or combination of both, depending on the disturbance caused by OCD in your routine activities. In general OCD needs long term treatment.

The medicines definitely work, but since you have discontinued in between, now you need dose adjustments or probably more than one drug. Continue CBT along with drugs if possible. Good luck.

There are many ways in which you can know you condition is getting better. One way to to find out if the anxiety is related to work and if you work functioning is as before or if it is declining. Another way is to find out from your peers and colleagues, if they have been seeing any changes in you since you have re started these medications.
