When you say since long, I presume it must be from late teens. That is when most disturbances reveal themselves. It may indicate that there may be some contributors from childhood. So it is better for you to visit a counselor with your parents and they can throw some light into your developmental years to help reach a diagnosis. The other thing is that you should now stop trying to work this out on your own because it will cause you quite a lot of discomfort and disturbance, and you will be wasting valuable time in the process. What you need to do is visit a counselor immediately and work out the emotional baggage that is bothering you to cause this anxiety, especially fear. Fear is the key for resolving the panic, successfully. These attacks can become more frequent and more forceful. There could be a lot of guilt that needs to be worked out. Panic attacks are curable but you need to comply, and with the complete cooperation that the treatment requires. Forewarned you should be forearmed. If the counselor thinks that you need medication, so be it. In the meantime get your body fit: it plays a very important role in order to harbor a fit mind. So exercise regularly, go for Yoga and meditation to train the brain, sleep well and eat healthily.
