My wife thyroid was 6.52, while she was taking thyrox 50 mcg (27/04/20). After that she started taking thyrox 62. 5 mcg and now she got pregnant but tsh at this time was 20.67. How much dose should be increased during her early pregnancy and what precautions should be taken?

During the gestational period of 9 months following levels of TSH are recommended : 1st Trimester : 0.1 to 2.5 mU/L, 2nd Trimester 0.2 to 3.0 mU/L & 3rd Trimester 0.3 to 3.0 mU/L. It is always good to maintain TSH around 2 mU/L through out the 9 months period. Therefore with onset of pregnancy usually 25% increase in the dosage is advised. So you may increase the same to 87.5 mcg per day. Then check TSH after 4 weeks. Please remember TSH needs to be checked every month during the gestational period. You have yo give her sufficient thyroid supplement to keep her tsh between 1 and 2 in first trimester. Please consult a physician/endocrinologist near you, who can evaluate her clinically and monitor the dose of thyroid supplement diring pregnancy and thereafter.

Hi you can increase to 125 mcg for 15 days rpt s. Tsh after 15 days .adv to consult endocrinologist and physician. Taking so many drugs for medical problem it is high risk preg.
