Hi doctor I am in 14 week of pregnancy. I was on thyronorm 125 since 1 years and till before 8 week of pregnancy when tsh came 0.1 doctor reduced to 75 mg since then and tested in between at 10 week it was 0.77 after 2.5 weeks it came 7.1 fasting tsh (test done without taking tablet and eating) next day I did retest at 1.30 pm non fasting tsh was 3.82 doctor asked to continue same dose as results vary and lessen value. Can anyone guide what should I ask doctor or do retest again and again. Is having 3.82 in 13 week with continuing thyronorm 75 is good option suggested by my doctor.

You must do free t3 and free t4. According to that, thyronorm dose will be adjusted. Tsh is an indirect indicator of your thyroid status, actual values depend on ft3 and ft4. You must never discontinue thyroid replacement without supervision as it's very essential in baby's intelligence.

Tsh value of 3.8 is fine. But sharp decrease or increase in thyronorm should be avoided. Since your tsh is 3.8 on 75 mcg thyronorm, continue taking it. Repeat tsh after 4 weeks of change in dose. Also in view of so much discrepancy, stick to one good lab and get your tests done from same lab everytime to avoid lab errors. Whenever you do, do fasting tsh before taking thyronorm. Hope this helps. Wish you good health.

Hello lybrate-user. Tsh values depend on the lab and it is prudent to get it done in a good qualified lab. Normally, in pregnancy it is good to maintain tsh below 3 as baby cannot produce its own thyroid hormones in the first 12-14 weeks and derives its source of thyroid hormones from the mother. It's advisable to take 100 mcg and get your tsh checked after 6 weeks and we can adjust dose accordingly. Make sure you get it done in a good diagnostic centre.

Yes tsh results vary in a day. So you have to get always morning fasting TSH As you have crossed first three months without any problem, most likely your thyroid is controlled. You can get retested after one month. Morning fasting TSH. And please repeat from one trusted lab. Don’t change labs. Hope that helps

Always do the test in the same reliable lab. Well controlled hypothyroidism should have a value of 2.5 miu/l.the fasting levels willalways be higher. What I suggest is 50 mcg of thyronorm daily and take a fasting sample always. Donot stop thyronorm optimum dose during pregnancy. You will have normothyroid cute baby.
