My grandfather, 80 years old, diabetic on metformin 500 mg (sugar controlled previously, not measured since 1 yr) had injured the lower part of shin bone by accidentally dropping a coconut. The injury resulted in a peeling of the upper layer with a 2x2 cm wound with mild bleeding. It has over a month now that the wound has not healed & the size has increased to 5x4 cms. He has started applying mupirocin oint since 7 days but not to any effect. The wound was left open right from day 1. I need advice regarding an oint which will improve wound healing in this case. I have asked a local pharmacist to do regular dressingof the wound from tmrw & also measure the blood sugar using glucometer. Kindly lend your valuable advice.

Blood sugar levels must be controlled strictly preferably with insulin. For wound management consult a general surgeon.

Thanks for the query. I have gone through the details mentioned. It is very essential to check his fasting, pp glucose and hba1c% immediately. If blood glucose is not in control then wound may not heal quickly. So first check that. If glucose is high then based on the level necessary change in medication needs to be done. Also check serum creatinine and urea levels. Then either come back here in a private conversation or consult your local physician or if you a surgeon dealing with diabetic foot please consult urgently. I hope you understand the severity of the situation. If you have any questions please do come back. Thanks.

It’s high time that you start him on antibiotics and proper dressing rather than just experiment. Fix appointment with a doc.
