My wife is taking istamet 50/1000 once daily at breakfast and having fasting blood sugar (10 hrs) 163. What should we do as her sugar is still not controlled. She also takes thyrox 75 once daily and losacar 25 once daily.

To suggest an upgrade I need to know following details: fbg of the day, pp glucose, hba1c%, weight, height, dietary pattern, extent of daily exercise. Lipid profile, age of the patient and since when is she diabetic as well as bp readings. Fasting blood sugar should be less than 126 and post prandial (after food should be less than 160) ideally. She might need a change in her dose or might need new medications. She needs to be reviewed by the physician. That being said, if there is a change in her routine i.e. She is getting less excercise or eating in an appropriate way that needs to be addressed.
