My fasting range is around normal whereas my pp (2 hrs after meal) is high (368). What should I do. Also note my other parameters such as sodium, chloride, hdl cholesterol are below normal range, do these have any effect in my diabetes range?

What is your fasting range blood sugars? You definitely need to increase your diabetic medications' dose or might need new add on medications as well as dietary adjustments. Need more information on hba1c, renal function tests lipid profile etc.
