My age is 47 I am a diabetic patient since 2004 its has been 17 years now I had consulted many doctors for the best treatment I had taken many medicines now I am taking gemer p2 twice a day and I am taking insulin (lupisulin m30) 12 units before dinner but my sugar level is not getting control I use to monitor my sugar level in about 2-3 days I checked that yesterday also and it was: fasting:175 and pp 280 could you please tell me what should I do, what precautions should I take.

I would recommend you to consult an endocrinologist and change your insulin from a premixed one to an analog mix like ryzodeg. However given the long-standing diabetes you are having I would suggest you to be on a basal bolus regimen which involves using two different insulins, in short two pens which will provide you finer control and better flexibility.
