Acute necrotizing pancreatitis post op diabetes. Operated in may'22 ongoing medications enzar hs 1-1-1, istamet 50/500 0-1-1 and ayurvedic medicine as alternative medicine. Currently the hba1c is 14.8 and blood sugar average is 347 mg/dl. Looking for allopathic medicine to bring blood sugar in control without insulin injections. Have severe allergic reactions with ct contrast. Have done approx 20-25 ct scans between 16 april 22 till date. Reactions were observed in last 3 ct scans during follow-up.

Hello, thanks for the query. I have seen the details mentioned. With h/o acute pancreatis there is no insulin secretion. Hence no oral medication can work. Because the oral drugs act either by stimulating beta cell to secrete insulin (by different methods) or by reducing insulin resistance. But because there is no insulin secretion at all so those drugs can not act. Hence, insulin injections is the only option. That to multiple doses may be required (long acting & rapid acting). The drugs like sglt-2 inhibitors like dapagliflozin, empagliflozin and canagliflozin may be tried as they function at kidney level. Thanks.
