Semen sample ko lene ke aadhe ghante baad hum check karte hain ki kaisa sample hai patient ko dikhate hain aur fir processing chalu karte hain. Fluid mein jo sub-sample hota hai vo niche aa jata hai. Upar ka fluid hum nikal dete hain. 45 minutes ke baad jo actively motile sperm hote hain vo load hoke upar aa jate hain. Natural and ismein kya farak hai? Natural intercourse mein semen vagina mein girta hai. Usmein se 40-50% bahar aa jata hai. Kuch bacha hua hota hai and jo actively motile sperm hota hai, unko pura travel karna padta hai aur fallopian tube mein aana padta hai. Aise egg ko fertilise karte hain and fallopian tube mein embryo ban jata hai. Aise hi vo uterus mein aa jata hai aur implantation hota hai. IUI ka success rate 10-20% hota hai. Patient to patient bhi ye depend karta hai. Ye low cost treatment hai and iske result bhi ache hain. Agar fallopian tubes mein problem hai ya fir block hai ya fir uexplained infertility hai jinmein sab kuch karne ke baad bhi pregnancy nahi rukti hai, unmein 4-5 times IUI ho jata hai but fir bhi pregnancy nahi hoti hai, aise patient mein hum IVF and ICSI advice karte hain. Usually hum ICSI karte hain. IVF mein embryo body ke bahar bnaya jata hai. IVF ke lia kya kya jaruri hai? Females mein har cycle mein 1-2 eggs hi bante hain. Hum menses aane ke baad 11-12 days tak female ko injection dete hain jis se hum ko jyada eggs milte hain. Hum iski monitoring karte hain, sonography mein hum ko samajh ata hai ki eggs kitne ban rahein hain and uske according dose jyada ya kam ki jaa sakti hai. 12-15 dine ke anadar follicle puri tarah se mature ho jata hai. Uska size around 18-20 mm ho jata hai.. Uske baad hum ovulation ka process start karte hain.

Ovulation hone se pehle hum sare eggs under anesthesia nikal lete hain. Hum egg kaise nikalte hain? Sonography ki guidance se hum needle prick kar ke follicle mein bhejate hain and fluid ke sath egg hum bahar nikal lete hain. Fluid ko hum embryology lab mein dete hain. Hum try karte hain ki 5-10 eggs humein milein. Agar aisa hota hai toh 80% cases mein embryo ban jata hai. Is se hum 8 embryo bana sakte hain. Embryo banne ke baad hum uski quality check karte hain and dekhte hain ki fermented hai ya nahi. Best embryo fir hum transfer karte hain. Agar jyada embryo bante hain toh hum 3-4 insert karte hain and baki ke embryos ko hum freeze karte hain future ke lia. Hum jyadatar patients ke lia ICSI hi karte hain and jyadatar cases mein IVF nahi kia jata hai. ICSI se humein ye bhi pata chal jata hai ki ucit 100% fertilise ho gaya hai. 8 hours ke baad hum check karte hain ki kitne embryos banne hain. Uske baad hum decision lete hain ki kitne embryos transfer karne hain. Iske baad hum 13-14 days ke baad hum test karte hain.

Ismein urine pregnancy test hota hai and agar vo raised ata hai toh aage hum medicine continue karte hain. Around 15-21 days ke baad humein samajh aata hai ki pregnancy kaisi hai like single hai, twin hai, multiple hai, cardiac activity kya hai aur uske baad hum decision lete hain. Iske baad patient ko similar centre par ane ki jarurat nahi hoti hai. Jahan patient ko convenient hota hai vahan ja sakta hai. Ismein pregnancy hone ke chances IUI se jyada hote hain. Abhi success rate kis pe depend karta hai? Patient ki age, eggs quality kya hai, embryo quality kya hai, uterus kaisa hai, linig kaisa hai, blood flow kaisa hai, patient ka luck kaisa hai ye bhi matter karta hai.
