Hi,

I am Dr. Suyansh Bhandekar, Gynaecologist. I will be briefing you today about infertility and its management. You must have observed that he infertility cases have been increased over the 1-2 decades. Science has also envolved into this to help the couples. As of ow 10-14% incidents are there in India. What do we mean by infertility? Infertility is the inability to conceive with unprotected and regular intercourse. We have seen 80% of the couples do conceive within a year. So, 10-14% requires some kind of potential. We divide infertility into 2 aspects whether it's primary where the lady has not conceived on time and the secondary where she has conceived and then she had a miscarriage.

Now she is trying again but unable to do so. So, what leads to infertility? When we look for the female factors which are almost the contributor 40-50% of the cases which involve your ovarian factors like PCOD, ovarian failure, endometriosis, a uterine factor which includes fibroid, polyp, etc or it could be a tubal thing where the tubal is blocked and have been damaged. 2nd the most important thing is the hormonal thing which deals with your menstrual cycle. Next is the malefactor. 40% of the cases we have seen because of male-related things. In which the semen parameters are damaged because of any injury, injections, testis, hernia or some sort of other problems. Sometimes it can be because of male and female factors both which we have seen in 20-30% of the cases. Last but not least, there are some factors which are unexplained. When do we approach a Dr for that?

10-20% of couples who are trying from past 2-3 years, they should definitely approach a Dr. Some couples do require early intervention. Just like the age of the lady is more than 35 or she has a history of the ovarian problem like PCOS, irregular cycles, a male problem which is hampering the problem. So, these patients should come forward and need to see a Dr as soon as possible. So, we advise very basic tests like hormonal tests, thyroid, prolactin, AMH, progesterone. Next is a detailed ultrasound which is a must. The only test which we advise for the male is the semen parameter or semen analysis which see the motility and morphology factors. If everything is normal then the malefactor is also normal. 2nd is tubal evaluation. We do the ovulation induction and IUI. 3rd and the last s IVF and ICSI which has the success rate of 60-70%. I am Dr. Garima Sharma, Gynaecologist. Today I will talk about the myths and facts of IVF. I will cover certain topics. What exactly is the IVF? Who requires IVF? Are there any side-effects associated with it? What are the success rates? Is there any difference between IUI and IVF? We make an embryo outside the womb by mixing egg and sperm inside the laboratory. Once the embryo is formed, the same will be transferred into the womb. There is a certain indication where IVF is the choice. So, in the first subset where you require IVF, where any female with a blocked fallopian tube or her tubes were removed because of any cause like ectopic pregnancy, damaged tube or there is any genetic issue with the couple or premature ovarian failure or the female achieve her menopause before the age of 40. There are other things also where IVF is considered as an option would be uterine issues, fibroids, unexplained infertility, miscarriage. Now the success rate of the IVF depends upon the multiple factors. So, the success rate depends upon the cause of fertility. What is the female age? How is sperm quality? What is the quality of the embryo? How is the uterus lining, is it good enough? And the most important is the laboratory condition where you IVF takes place.

What are the side-effects of IVF? Thanks to the advancement and sciences of the technology and the protocols that we have. Side-effects of IVF are very much limited. The issue which is concerned is multiple pregnancies. It all depends upon the better freezing technologies and laboratory conditions. Is there any difference between IVF and IUI? In IUI, the processed semen from the male partner is inserted directly to the womb and in IVF, we take out the sperm and the eggs, fertilize the same in the laboratory. Now the last part of my talk is myths about IVF treatment. People think that IVF is the solution to all your fertility and it will give you 100% success rate. So, as I already told you that the success rate depends upon the multiple factors. The average success rate is between 30-65% across the globe. Now the next myth that the baby which are formed with IVF have some congenital issues or they are malformed babies. But malformed babies issue can also happen in normal pregnancy. In IVF, you have to take the bed rest, but it is a daycare procedure and it does not mean any kind of bed rest.

You can resume your normal activities once the procedure is done. Then people think that IVF is meant only for rich people. So, yes, IVF is costly if you compare with other modalities. So, it is the treatment of choice. Next people think IVF means C-Section. C-Section depends upon your antenatal course and it's being decided by your gynecologist.
