A detailed assessment would be required in order to understand the reasons why you feel you are at fault. If the problem is primarily depression then you may experience irritability, low self esteem, and negative thinking thereby blaming yourself for even reasons beyond your control. For this you will benefit from being on medication to improve depression. On the other hand if there are certain personality traits that you need help to improve then you should seek counselling and therapy. In the meantime make a written note of situations (like a diary) which make you feel you are at fault and reflect on areas of improvement.
