Mr. lybrate-user Verhese, Thanks for the query. The blood glucose levels mentioned are seriously high. It is absolutely essential to bring those under control very quickly. Ideally fasting should be < 100 mg, PP 150 to 160 mg & HbA1c5 <6.8. With such high levels there is a severe resistance to yur Insulin action due to glucotoxicity and lipotoxicity. Therefore, it is better to initiate the treatment with Insulin injection along with oral drugss, achieve a strict and good control of blood glucose. Then after about 3 to 4 months we can discontinue Insulin and control the blood glucose with oral drugs, diet and exercise. If you are only on oral drugs now, it will be difficult to achieve a quick and necessary glucose control. As regards the diet, I need to know the present dietary pattern, extent of daily exercise and drugs (if already treatment has been started). Following are certain precautions to be taken: 1) Stop all sweets, deep fried foods, avoid junk foods, pastries, bakery items and all foods made from maida. 2) Increase the intake of green leafy vegetables, salads and fruits. 3) Always use whole grain flour for roti and chapati. 4) Avoid fructose syrup containing foods, colas and red meat. 5) Refrain from alcohol and smoking. 6) Exercise daily for over one hour.
