Long standing diabetes causes neuropathy I.e. Nerve damage in feet and vascular disease I.e. Blood circulation problem. Both can cause foot pain in diabetes. Go to an endocrinologist or experienced physician for consultation and get some tests like HbA1c, B12 level, Vitamin D level ,Peripheral Doppler etc.
