TB Gold/TBFeron test does not differentiate between TB infection (mere presence of TB bacteria in the body) or Active TB disease. Just by appearance of pleural fluid you can not say that it is tubercular or not. Some times whitish appearance may help to know that fluid is chylous or chyliform but tests are required. Other tests like AFB smear, AFB culture, Biochemistry, cytology for malignant cells, LDH, ADA, GeneXpert (though not commonly recommended due to low sensitivity but if positive it make diagnosis certain). You might need more tests and work up for B/L pleural effusion. Your history of exposure to certain substances like Asbestos may also help which might be linked to your job or work.
