Dr, I was taking glycomet sr 500 mg twice daily after meals for the lady two years. My hba1c level was fluctuating between 8 & 9. 8.4 as the average. But, in the last 6 months, the levels have shot up to 9.5 & 11.4 this month. Hence, when I consulted a diabetologists in my area, she asked me to change the drug, and prescribed glycomet trio 2 mg twice daily with neurobion forte. Please suggest if it is advisable to directly jump from a single drug usage to a combination of three drugs. And, is this the right combination for me. Am 48 years and weigh 78 kg, and follow a diet & daily exercise regimen. Appreciate your advice & recommendation. Thanks.

Hi Mr. lybrate-user. The above said treatment would be completely apt for your levels of hba1c. Since the blood sugars have drastically shot up shows that the body has developed insulin resistance and that one pill is not sufficient to push the sugars from blood to cells. Thus 2 primary drugs with a add on drug in a single fixed drug combination is very apt. Good for your compliance too.

Please understand when diabetes is properly treated with adequate dosage of drugs plus restricted diet and regular exercise along with maintaining near normal weight, glucose levels should be: fasting < 100 mg or closer to 100 mg, pp 150 to 170 mg & hba1c% < 6.8%. That is considered as good control. This helps prevent long term complications associated with diabetes (affecting eyes, peripheral nerves, vessels, kidney and heart). Therefore it is essential to achieve a strict glucose control. In your case there seems to be absolutely no control of blood glucose at all. So if the treatment has been upgraded by your doctor it is very much essential. To me it is still less. You need to be put on insulin injections at least for 3 to 4 months to bring glucose under good control. Otherwise you are risking your own life. Thanks.
