I am 64 & diabetic for 20 years. Present dosage 2 gms of metformin and 4+2 mg glimepiride in a day. Random was 143 & fasting 191 today morn. Can I add 25 mg of acarbose to it?

No benefit as your fasting blood sugar is high best is start inj. Lantus 10 units at night before going to bed. More discuss on line with me.

Since your you have long standing diabetes and adding on acarbose my not achieve the target in my opinion basal insulin will help you more then acarbose.
