I have been grappling with the problem of ocd and anxiety for last 5 years. I have been taking paroxetine 75 mg and olanzapine 5 mg. Actually I am not responding to antidepressants well, and my treating doctor wants me to shift to new maoi's..thus will the maoi's be able to manage ocd as like paroxetine? I have dysthymia as well. So is it worth switching to maoi? Will it be able to manage ocd thought? Thank you.

Dear Ankur, Shifting to new MAOI may be good for your dysthymia. What is your major problem, OCD thoughts or rituals or low mood? If it is OCD thoughts and ritual, it is better to try with other SSRI like Fluvoxamine or Clomipramine. Probably your doctor has already tried them before paroxetine. CBT with exposure and response prevention (ERP) gives better results with SSRI. Have you tried?

Hello, I wouldnt advise Maoi unless its a last resort. It has a lot of interactions and side effects with food and other medicines. Normally This is the protocol followed in sequence SSRI (u are taking this as paroxetine) SNRI TCA any of the above primary antidepressants with a low dose mood stabilizer any of the above with a low dose atypical antipsychotic combination of all 3 Addition of mirtazepine to the above etc etc After all these protocols are done only then you should consider MAOI's in my opinion sir

Dear lybrate-user sorry to hear that there is no much improvement. As such ocd is difficult to treat because individual response to medication is different, and medicine working for one person doesn't work same way to other one, and individualization of correct medication is needed. I don't know how many medication has been tried or whether your illness meet the criteria of treatment resistant ocd. If current medication not working at all than changing medication is only option. So far mao is concerned, if other ocd medication trials been done, than trying is not a bad idea. Only thing you should be cautious is avoiding some food, please clarify if your doctor prescribe it than what food should be avoided. Be safe, get well soon.

Hello sir, maoi have certain side effects, but if you are not responding to that ssri it would be better to shift you from that. These days we have been using new combination of serotonin modulator with aripirazole, they have better outcomes thanks.

Dear lybrate-user, you are on decent doses of a good anti ocd medicine, but unfortunately rates of response in ocd can vary. Your consulting doctor maybe moving to treatment resistant protocol. Kindly follow the doctor's advice. I assume you were tried on tca too earlier.

Hi lybrate-user, pharamcology (medicines) by themselves may not be sufficient for ocd and anxiety issues. Consider seeking cognitive behavior therapy from a clinical psychologist which would work directly on your behaviors and thoughts while the medicines work on your anxiety. Cbt is very efficacious for ocd and you will see good results.
