Hi doctor. My mother is suffering from severe pain in lower back that intensifies and spreads to her ribs during night. She has long medical history. She had been treated for breast cancer in 2013. With surgery followed by chemo, radiation and nonsteriodal medicine of valodex 20 mg. Then again in 2018 she developed metastatic cancer in her lower back near spine. The treatment of which started with radiations followed by hormone therapy which is still going on. Our oncologist had prescribed her letroz 2.5 mg.gabapin 300 mg. Shelcal 500 mg. Apart from it she has taken one cycle of palbociclib. But due to its very high cost and our non-affordability towards it we have to discontinue it. She has been diagnosed with pet ct scan several times but results were always good. With disease still growing in body. So my question to doctor is that. Whether we should perform mri scan for assessment of cause of pain. Or it may be cancer redeveloping. Which could be recognized by biopsy test. Followed by radiation therapy as in the case like in 2018 after which she felt alot relief. From similar pain. Or we should only continue with our hormone therapy including ribociclib. Ie alternative cheaper medicine suggested by our Dr. against palbociclib. And one thing. She is taking zoledronic acid inj every month as prescribed by our doc for protection of bones from osteoporosis and cancer. Which is postponed by every 3 months by doc. On examining the good pet scan reports. So please guide me doc. What treatment strategy should I adopt to get her well.

Your mother's low back pain with radiation to ribs is most probably because of the tumour spreading to the spine; she might have suffered a vertebral collapse fracture or could be a pain resulting from increased tumour activity in the area. Whatever be the case, she must get an urgent mri screening of spine done in order to elucidate any complications that might have occured. Further treatment would be decided based on mri findings. For the time being continue the ongoing medical treatment. Review with the mri films and report.

First of all I’m so sorry to hear about your mum. As she already had metastatic cancer before and there is a high likely chance of pretty much the same in the Thoracic spine which can give symptoms in the lower back spreads to the ribs. I looking in the picture now she definitely needs an MRI scan of her spine to rule out further metastasis. Depending on the MRI scan we can decide what will be the best treatment for her. Most likely to treatment might be like a palliative care. I know it’s very difficult situation from your point.
