Fasting is 8 to 14 hours no calorie intake. At 4 am sugar may be little high, google dawn phenomenon try fbs at 6 am. Again high try to reduce carbohydrate serving at night meal.

You should not be worried about your fbs as long as your hba1c is normal. However tiban is not the basic medicine for diabetes and not very helpful for controlling fasting glucose. Kindly consult for further advise and alteration in medications.
