However, there is no mention following information: hba1c%, fasting & pp glucose levels, weight and height, dietary pattern and extent of daily exercise. Plus what is the age (is it 45 years as mentioned)? Among the drugs being taken by the patient there is glimepiride 2 mg with metformin. Glimiperide is comparatively a long acting sulphonylurea (su), therefore, in older aged people there is a possibility of developing low blood glucose that is blood glucose going < 70 mg. So 40 mg & 45 mg is too low. This is not good for the patient. So one is please come back with all the details that I have mentioned above. Presently you can stop glimiperide and then look at the glucose levels. Or at least dosage of glimiperide has to be reduced to 1 mg a day. If you have any questions please do come back.

Iif blood sugar reduces only in the evening then reduce the dose of glimicor m2 to glimicor m1. Get her fasting, pp and at bed time blood sugar test also done.

She may have to change to a preparation with glimieride of 0.5 or 1 mg, follwo the dietary advice below and then monitor sugars as follows reduce the quantity of carbohydrate (rice, wheat, rava, maida) in each meal and increase the quantity of protein (dhal, egg white, fish or chicken. Monitor blood sugars with glucometer as follows. Fasting and pp on wednesday and 2 hour after lunch and after dinner on sunday and maintain a record. Target lood sugars fasting 80-120 post meal 140-160 for someone her age. Monitor hba1c once in 3 months. If the low sugar problem persists seek a consultation to sort out the problem
