However, there is absolutely no information on present fbg, pp glucose levels, hba1c%, extent of daily exercise, complete dietary pattern (from morning till dinner), serum creatinien, urea and lipid profile. Unless all the necessary details are given there is no way to comment. In a well controlled diabetic patient fbg should be < 100 mg or closer to 100 mg, pp 150 to 170 mg & hba1c% <6.5%. If you have been losing weight on metformin and glimiperide and still your sugars are out of control on your usual diet, it is likely that you may need to consider adding insulin after a proper consultation. In the meantime try the following change in diet and then get back to us with your home glucometer monitoring results say after 4 weeks if your fasting values are > 120 and pp values go above 180, reduce the quantity of carbohydrate (rice, wheat, rava, maida) in each meal and increase the quantity of protein (dhal, egg white, fish or chicken. Monitor blood sugars with glucometer as follows. Fasting and pp on wednesday and 2 hour after lunch and after dinner on sunday and maintain a record. If in spite of dietary adjustment your sugar levels continue to be high fasting > 120 and pp more than 180 check your hba1c also and seek a consultation to adjust your anti-diabetic drugs or to add insulin
