I had moderate white pustules on my scalp, chin and chest. I was prescribed different antibiotics in different consultation each time but the acne (pustules) seems to come back after stopping the antibiotics. At last, I was prescribed 20 mg isotretinoin for 2 months. It worked well for the chest acne but the ones on scalp and chin have started coming back (although not that much as previously). Was the dosage of my isotretinoin less? (my weight is 70 kgs)

Hello hope you are keeping safe 1) from your history, I understand that you are having bacterial folliculitis with truncal acne. Once prone to folliculitis, there will be a recurrence of it. With medications, these subside and on stopping it may tend to come. 2) retinoids do work nicely for the truncal acne and 20 mg is the safe dose that works. Ideally it is continued for 3 months and then blood levels are repeated, however you can follow what your dermatologist have said. 3) you can use - for scalp you can use some antibacterial washes with an anti-fungal coverage - a non soap cleanser for face - antibacterial ointments like clindamycin for the acne/ pustules on face - antiacne gels like benzyl peroxide for the the ones on the face - use an oil free moisurizer -have antoxidant tablet daily for a month kindly note the frequent shaving can increase the occurence of these folliculitis, so decrease the frequency and always shave in the direction of hair growth. 4) for a specific treatment and dosage, kindly send me a photo of the lesions so that I may be able to guide you better. 5) please be aware that acne break outs could be due to various reasons like - hormonal imbalance - stress - excess humidity - environmental pollution - excessive sebum/oil production - dandruff / pityriasis capitis - excessive moisturizers on face - oil based cosmetic products - frequent touching of face with unclean hands - an improper sleep wake cycle - junk diet and foods like egg, dairy products, meat and poultry food, nuts and oily food - an underlying disease like pcod and thyroid - hereditary - dehydration - excessive sun exposure 6) also take care an be aware about few of the reasons that causes these acne / pustules - high consumption of dairy products - cola, caffienated drinks, aerated drinks, high sugary foods - high humidity - over application of moisturizers and creams clogging the pores - excessive sebum secretion inherently. - picking/ squeezing acne, abrasive washing or frequent scrubbing revert back with a photo for specific treatment and dose hope your query is cleared thank you and take care.
