Thank you for your reply. I have already increased my novomix 30 to 24 units. Still pp after breakfast is 210 and pp after lunch is 249. This 24 units causing severe hy condition at 12 noon. I am not using any medicines since they are causing my muscle loss at thies area. If I use nova rapid 3 times and long acting at once in the night, total 4 punchings will be there. But entire life will be with always using insulin only. Pl advise for ideal solution. Regards prabhakara rao v vijayawada.

Muscle loss is due to faulty injection technique if you daily donor change place of injection within a site say abdomen this issue will be present you definitely need insulin you can use injured tougeo or glargin at bed time say 20 units and use injured novomix 30/70start with 6 units before breakfast and 4 units before dinner and adjust dose every 72 hours. It is difficult to give a sound advice based on what information is provided. I need to know following details : FBG, PP glucose, HbA1c%, dietary pattern, extent daily exercise, lipid profile, serum creatinine and urea. Plus let me know why no oral drugs have been used? Were you earlier on any oof those earlier? If ues what were the drugs and dosages? Another aspect in this question (open ) I can not suggest any drugs, can only give general guidelines. Dear I suggest that you take a light snack at arond 10 to 10 30 am to avoid the hypoglycemia before lunch. For fine tuning diabetes control try the following if your fasting values are > 120 and pp values go above 180, reduce the quantity of carbohydrate (rice, wheat, rava, maida) in each meal and increase the quantity of protein (dhal, soya, egg white, fish or chicken). Monitor blood sugars with glucometer as follows. Fasting and pp on wednesday and 2 hour after lunch and after dinner on sunday and maintain a record. If in spite of dietary adjustment your sugar levels continue to be high and if your hba1c is more than 8 % seek a consultation to adjust your anti-diabetic drugs regards, Â
