Hello, thanks for the query. When one is on adequate dosage of levothyroxine, tsh is well controlled and does remain around 3 to4 mu/l. Tsh 1.85 mu/l is quite good. However, the present value 22 mu/l is too high. It is usually unlikely for such a rise if one is taking levothyroxine correctly. No exercise or other things hardly affect tsh. So it is difficult to answer your question why the sudden change. Please let me know following details: weight, height, extent of daily exercise, lipid profile. Plus if you have any doubt please get the test repeated. After that if the test does come high then there is a need to increase the dosage of levothyroxine. Thanks.

Dear lybrate-user, you were only on a small dose of thyronorm upto now and this means that your doctor felt that the tsh was only mildly elevated in the beginning probably about 10 uiu/ml. If your tsh has suddenly gone up it may be because 1. You may have forgotten to take your tablets for a few days in the last 1 month or 2. Your hypothyroidism has become more severe. I suggest that you take 25 ug daily on an empty stomach without forgetting and recheck tsh after 2 months. If you forget the tablet on a particular day you can take it later in the day when you remember. If after 2 months your tsh is still high in spite of regular intake of tablets, get an online consultation to adjust the dose
