Recently I consulted a gynecologist, who has prescribed glycomet sr 500 for weight loss & pcos. Will I have to take the medicine for lifetime if I start taking it. My insulin & fasting, pp blood glucose, fsh, lh are normal. I have hypothyroidism, facial hair, dark patches on neck, armpits. Weight: 90, height: 5'4"

It need not be a lifetime medication. Once the problem got resolved, your current physician will decide upon it. Their are some medicine which works for both diabetes and pcos, and it will do good eventually. The most aspect of your treatment is to loose weight. Dear lybrate-user, for your height of 164 cm the ideal weight would be 60-64 kg and so at present you are overweight by about 25-30 kg. The pigmentation (dark skin) over the neck and armpits suggest that you have insulin resistance. The increased facial hair is probably because with insulin resistance, there is a tendency for the ovaries to secrete more of male sex hormones. Usually this also causes irregular periods. With all this, it is important for you to bring your weight down to about 65 kg gradually say over the next 3 years. The key to achieve this is by a very low carbohydrate normal protein diet and 1-2 hours of exercise/day. The hb a1c of 6.1 indicates a tendency for diabetes and this will be reversed by weight reduction. Metformin is often used int his situation but is probably inferior to diet and exercise. I do not know about how severe your hypothyroidism is and how well controlled it is but it is important to maintain a tsh of less than 4 uiu/ml for optimal health

Hello, Thanks for the query. I have seen the details given. You have PCOS plus with a BMI(Body Mass Index) of34.06 kgs/Sq meter (Normal < 23 kgs/Sq meter), you are already close to grade 2 obesity. Your ideal body weight should be about 61 kgs, which means there is an excess of almost 30 kgs. This is the main culprit for PCOs as well as prediabetes. Metformin given may help in minor weight reduction, but it does have benefit in PCOS. If you do not work seriously on weight reduction, then within next few months you can get diabetes. Since you have hypothyroidism, it is necessary to maintain TSH around 3 mU/L, then only efforts towards weight reduction may bear fruits. All the complaints mentioned are related to hypothyroidism and partly insulin resistance due to excess weight. There is no mention of the age. My suggestion is please do regular exercises for over 1 hour a day. Preferably join a gym and do work outs under the guidance of a trained professional. At least 8 to 10 kg weight reduction will alter the picture. Thanks.
