" hi there, I am a medical doctor myself and an mbbs graduate, who is preparing for neet-pg myself, so do not have much time rather two months left before exam ,so this is highly directed towards only senior psychologists (preferred) and senior psychiatrists, I have a psychiatric history of using medication since 4 years and right now I am having occasional crying spells and inability to feel like reading which I used to enjoy before although I am put on bupropion 300 mg and lamotrigine 200 mg and amisulpride 100 mg and alprazolam 0.5 mg ,sleeping meds are just to counteract modafinil which I take them occasionally to decrease fatigue and increase motivation to study, I will be thankful if someone who is well experienced can provide me with counselling to decrease to cope up with stress and crying spells and any one who can alter medication to the circumstance I am in right now can just give a suggestion, I want to a consult with a psychologist or a psychiatrist and I do not have enough time to maintain a mood journal so any coping skills within short time against crying spells can just drop in a suggestion ,i want to take a consult, thank u.

Dear Dr. lybrate-user, it is nice of you to seek counselling also for coping with your mood. You can not adjust the dose as you like because none of these tablets work immediately to solve your problems. Only exception is alprazolam that acts immediately to reduce anxiety. When tablets themselves take time to act, how can counselling can make you to study for your exams well? Any form of cbt takes minimum 5-6 months. Mood journal will help with your study schedule. There are hours that your mood is better (usually by evening), there are days of good mood (usually after periods). You have to cash on those days and hours to study. Don't force yourself, if you are depressed. Counsellors will also not motivate you during your depressed mood. They can only help you to learn to live with mood disorder better. Let crying spells come and go. Be sure it will pass off (of course, it will come back)

Thanks for contacting. Time. Is less but yes something can be definitely done to help you cope. Details of your condition shall be required though, I am assuming that you already have read about your previous condition and current condition as well. And what options you have. If I make a suggestion will you be able to do it? So if suddenly you start crying can you actually cry out loud full hearted without thinking about wastage of time or what others will think without suppressing it? Try it. Let all your emotions come out vent out as much don't control it stop it. Cry. Notice if you feel lighter. Stress is generated more because you want to avoid it and avoid the thoughts feelings associated with it. Ofcourse exam stress is there but your condition is also generating stress unconsciously. If you can take out time it can be 10 min or 30 min max. Or 5 min in between your studies, perform an activity completely opposite of studying. Sing a song, broom your room, do deep breathing or pranayam, take a glass of water and just walk around your home, make yourself tea/coffee. Anything which is absolutely different from studying. You can even talk with your family or friends. I don't know if you have heard this but you could do mindful observation, a very good technique to remain in present with least amount of interfering thoughts. At the end of the day medication is definitely helping you to keep the chemical balance but to maintain a holistic balance you have to try other alternative methods. Unfortunately, therapy cannot be hastened in any manner but it is possible to bring about some functionality to begin with and gradually you can develop good coping skills and recovery over time.
