In 2018, I have detect pre- diabetic and take medicine glyciphage sr 500 on doctor consult. On regular of fasting always 90 to 120. But recently I found in sep 2020 test, my hba1c is 8 and fasting and after meal 127/198. I have start dynaglipt m forte 20/ 1000 mg from october onwards. And watch my sugar level not down always above 135 / 190 without medicine and with medicine avg 120 / 170. Kindly suggest for further monitoring. Also I have hear it is reversible?

Hi diabetes has a quite variable spectrum and we don't know where you sit currently. It might show improvements and deteriorations and it all depends on how much insulin your pancreas can produce and how efficiently the insulin can act. The medication which you use now will help to increase your insulin secretion and decrease your glucagon secretion and also increases glucose utilisation of the body. Glucagon is a hormone that is released to increase the blood sugar. You should be probably having type 2 diabetes for which these medications work. Insulin is like a key- you need this key to open a gate which helps glucose to enter from the blood into most of the tissues. An interesting fact is that even if your body produces enough insulin it can't decrease the sugar levels in your body. This is called insulin resistance. This means even if you have the key the lock doesn't open and it has become a bit faulty. If you do regular exercise the lock may start acting better and insulin can play it's role. Another fact is that if your blood sugar becomes high after a carbohydrate rich meal these locks can become faulty again and the body will start producing more insulin thinking that it needs more of it. No matter how many keys you have, if the locks are faulty you can't open them. So when you decrease your carbs the total insulin needed for your body will decrease. Your pancreas always will be running with the maximum speed to produce insulin. But after a while like any engine it gets broke. That is, it can't produce enough insulin. So the insulin resistance initially and defective insulin production towards the latter half of the story makes it hard for your body to regulate blood sugar. If you don't check the pace it will eventually result in a total breakdown of your pancreas where you will have to take insulin. This process is not reversible although you may find improvements and this doesn't really mean that it is cured. It is an ongoing process. If you stop using the medication and doesn't choose a healthy lifestyle you will have to pay a huge price in the end. It could affect your kidneys, heart, nerves of your eye and almost all the nerves. Ideal target will be a fbs less than 130 and a sugar level after food between 140-180. The idea is to limit the fluctuations in your blood sugar patterns more than one single value. You will need your hba1c tested once in 6 months a yearly creatinine and electrolytes for kidney function and a yearly ecg as well as eye check. Also your blood pressure needs to be below 130/80. Please understand treating high blood glucose doesn't mean just taking some tablet on your own. Taking tablet is just one third of the treatment, diet and exercise as well maintaining near normal body weight. In a well controlled patient fbg should be <100 mg or closer to 100 mg. Pp should be 150 to 170 mg & hba1c% <6.5%. Looking at your bmi which is 25.62 kg/sq meter (normal <23 kgs/sq meter) which means you have over 7 kgs extra. There is no mention about your diet and exercise, which play a major role in controlling blood glucose. Plus we need to know lipid profile and bp.
