I am a 22-year-old female. I was diagnosed with hypothyroidism in 2015 and started carbimazole. My f.t4 was 18 ng/dl in 2015 but now it is in the normal range (1.74 ng/dl). But my tsh level is around 0.007 microiu/ml since 2015. My doctor suggested destroying the thyroid gland with radioactive iodine. My questions are, 1. After destroying the thyroid gland, do I have to take thyroid hormone externally? 2. Will it affect my day to day activities? 3. Will I be able to get pregnant in 2022? Thank you in advance!

After destroying your thyroid you will have to take thyroid hormone.Pregnancy is possible in future with keeping you euthyroid(normal thyroid functions).

Hello, Thanks for the query. I have seen the details given. With hyperthyroidism usual treatment is to take anti- thyroid drugs like Carbimazole for a period of 18 to 24 months. If by that treatment thyroid functions returns to normal (TSH in the normal range 0.5 to 5 mU/L & T4 <12ug/ml) then treatment is discontinued. In quite a few cases thyroid function does remain normal. No further treatment may be needed. However, in many cases the excess production of T4 returns and TSH goes down. In such a situation two other options of treatment are 1)Thyroid gland removal or 2)Radio Iodine (RAI)Therapy causing ablation of thyroid. In your case it appears that already almost 5 plus years have been given for correction to occur. But the hyperactivity still there. In such a situation RAI is a better option. After that you will become hypothyroid that is T4 will be less. This will increase TSH levels above normal. So you will have to be on adequate dosage of Levothyroxine (T4) for life time. Dear lybrate-user, as you are planning on a pregnancy by next year, it may be wise to continue only carbimazole for control of hyperthyroidism. As soon as pregnancy is confirmed by urine test change over to propylthiouracil for the first 3 months of pregnancy. After this you can change back to carbimazole. During pregnancy the requirement for the tablets usually comes down and the decision regarding radioactive iodine therapy may be taken say a year after delivery
