I am 75 year old male. Diabetic from past 20 years. My fasting blood sugar is 280 even after taking reclide xr 60, forxiga 10 and glimepiride 2 mg in morning and 1 mg in evening. I have recently switched from galvus met 50/1000 to said medicines as my creatinine was 2.1,kindly suggest which medicine should I take now.

Hello, thanks for the query. I have seen the details mentioned. The fasting glucose is oo high, as it should ideally be around 100. Pp should be 150 to 170 mg and hba1c% < 7%. (those two values are not given, must be pretty high.) with creatinine 2.1 mg/100 ml, there is a problem with kidney functioning. At this level and at 75 years of age his egfr (which is only 20.9 ml/minute) is very low. So it is possible that drug like metformin can not be given to him. He needs to be shifted on insulin injection urgently. So please come back in a private conversation with following details: weight, height, pp glucose, hba1c%, serum urea, lipid profile, dietary pattern and extent of daily physical activity. Or else please consult your local physician as he needs to shifted on to insulin like glargine once a day along with some of his oral drugs. I would suggest you to stop forxiga as your creatinine level is on the higher side. When did your creatinine start rising? You will have to switch to insulin to prevent worsening of your kidney function. Rapid acting insulin after each meal with a small dose of basal insulin will be ideal for you. Also need to keep a check on blood pressure and blood cholesterol.

U will need a change of medicines. Pls check your after meal sugars as well, so the doses can be adequately adjusted. You may need a small dose of insulin which will benefit the kidneys
