My father is 82 years old and has been suffering from senile dementia and insomnia. Currently he is taking the medicine clonazepam 0.5 mg + escitalopram 10 mg for insomnia. Last one month his serum sodium level fell below 125 meq/l. To control hyponatremia, doctor prescribed him to take tolvaptan 15 mg with 05 g salt capsule daily with a water restriction of max 1200 ml per day. After (tolvaptan + extra salt + water restriction) for last 20 days, his serum sodium level is now 136 meq/l which is still below the normal ref value 138 - 148. Should we continue tolvaptan 15 mg + 5 g salt cap + water restriction for more 20 days or increase the dose of tolvaptan to 30 mg? Please advice.
