Hi. I have been diagnosed with panic attacks and mild depression. My doctor started me on mirtazapine two months back and increased my dose to 15 mg which is going steady. He also prescribed me clonazepam and eventually reduced the dosage. He started me on tofisopam 50 mg twice daily one week back. I was fine taking the tablet for the first four days and I was fine. After 4 days I started developing some weird dizziness, spinning inside my head, confusion. Then after one week I stopped taking tofisopam for two days and mirtazapine for one day. And the dizziness has only increased but not improved. I again started tofisopam 50 mg and I find that my dizziness has gotten little betterbit it persists but I have a headache. I do not understand these drugs. Can anybody explain it to me whether its advisable for me to continue or how do I get rid of this dizziness and inability to focus.

Dear lybrate-user, have you thought that dizziness and inability to focus are symptoms that you had before the tablets were taken. Please don't jump to conclusion that they are due to tablets. No need to mess up with irregular use of tablets. Especially mirtazapine needs to be there daily to enter into your brain and start its action. Stopping in between, delays its antidepressant action. Drugs like clonazepam and tofisopam act immediately and develop tolerance soon. It means, it will reduce panic for few days, later the same dose will not be enough to get the same effect. You need not understand these drugs. It is for your doctor who has studied for more than 10 years of pharmacology and other subjects in mbbs. Â

Madam, the symptoms which you expressed over here can be caused by both your medications. You will have to consult your psychiatrist and he will adjust your medications in context you your initial and newly developed symptoms. Don't worry these symptoms will disappear with adjustment of medication. Â

Hi lybrate-user, mirtazapine is an antidepressant which has anti anxiety properties and used as anxiety medications. It could be used on long term. Clonazepam and tofisopam both are benzodiazepines which could be used on short term basis for anxiety but the side effects are dizziness, sedation and can develop dependency. Â

Tofispam is an anxiolytic. You may not have been able to tolerate it. It is not anyway first of even second choice anxiolytic. Mirtazapine has both antidepressant and anxiolytic effects. Clonazepam again is used for short term anxiety control. Based on what you say discuss your issues with your psychiatrist or get a second opinion from an astute psychiatrist. Good luck. Â

Dear sir, I strongly advise you to discuss it with your doctor. Only the doctor who prescribed you medicines is at better judgement of dosage of change of medicines and never ever stop medicines without consulting your doctor. Depression also needs psychotherapy along with medicines for complete cure. I advise you to consult a therapist for counseling. Â
